I understand this is NOT anonymous — my image and likeness may be recognizable — and it is separate from any anonymous impact reporting I may complete.*
I understand this is completely voluntary. Saying no will not affect any services I receive from RR&B.*
I understand I may withdraw this permission at any time by emailing Partlowsr@reentryrecoverybeyond.com. Withdrawal stops FUTURE use of my photos/videos. RR&B may not be able to retrieve materials already printed, distributed, published, submitted in a grant report, or reposted by third parties.*
